Health, Behavior, Lifestyle Treatment (HBLT) Program
The Health, Behavior, Lifestyle Treatment (HBLT) program is a comprehensive, patient-centered approach designed to support individuals in improving their overall health and preventing chronic disease. With over 50 active participants, HBLT addresses the critical need for ongoing guidance, medical evaluation, and lifestyle intervention among patients who are at risk for developing comorbid conditions such as heart disease, diabetes, liver disease, and hypertension.
The program integrates clinical consultation, routine bloodwork, personalized follow-up, and evidence-based use of medications when appropriate. In addition, it emphasizes the importance of sustainable lifestyle changes, including physical activity and nutrition, supported by tailored resources and coaching.
HBLT is built on the recognition that many patients require more than just episodic care—they need a structured, proactive, and supportive model to help them make lasting changes in behavior, health, and overall well-being.
